A drug interaction occurs when two or more substances โ drugs, foods, alcohol, or supplements โ affect each other's activity in the body. The result can be that a drug becomes more powerful, less powerful, or creates new effects that neither substance would cause alone.
Drug interactions are one of the most common and preventable causes of medication harm. The NHS estimates that adverse drug reactions account for around 5-8% of hospital admissions in the UK. Understanding even the basics of drug interactions significantly reduces this risk.
Types of drug interaction
1. Drug-drug interactions
The most common type โ when two prescription or over-the-counter medications affect each other. Examples: ibuprofen reducing warfarin's metabolism; tramadol and SSRIs causing serotonin syndrome; gabapentin and opioids causing respiratory depression.
2. Drug-alcohol interactions
Alcohol interacts with dozens of commonly prescribed medications. Some interactions are mild (increased drowsiness with antihistamines); others are severe and potentially fatal (metronidazole causing disulfiram reaction; benzodiazepines + alcohol suppressing breathing).
3. Drug-food interactions
Certain foods affect how drugs are absorbed or metabolised. Grapefruit juice inhibits CYP3A4, raising levels of many drugs including some statins. Vitamin K in green vegetables opposes warfarin. Dairy products bind tetracycline antibiotics, reducing absorption.
4. Drug-supplement interactions
Herbal supplements and vitamins can interact with medication. St John's Wort is one of the most potent โ it induces CYP enzymes and reduces the effectiveness of many drugs including contraceptive pills, warfarin, and antivirals.

Pharmacokinetic interactions
These affect how the body processes a drug โ how it is absorbed, distributed, metabolised, and eliminated. The most common mechanism involves liver enzymes (particularly the cytochrome P450 family, including CYP3A4, CYP2D6, and CYP2C9). If one drug inhibits or induces these enzymes, it changes the blood levels of another drug.
Examples: fluoxetine inhibits CYP2D6, making tramadol simultaneously less effective and more toxic. Clarithromycin inhibits CYP3A4, dramatically raising statin levels and muscle damage risk.
Pharmacodynamic interactions
These occur when two substances have additive, synergistic, or opposing effects on the same body system. Examples: ibuprofen and alcohol both irritate the stomach lining โ together the damage is greater than either alone. Diazepam and opioids both suppress the CNS โ together they can stop breathing.

Why "generally safe" still requires care
Individual variation means "generally safe" does not mean "safe for everyone." Kidney function, liver function, age, body weight, genetic variation in drug-metabolising enzymes (pharmacogenomics), and the specific doses involved all affect whether an interaction becomes clinically significant.
A combination rated safe at standard doses may become hazardous at double doses. A combination fine in a 25-year-old may be dangerous in an 80-year-old on 10 other medications.
