Is it safe to take Ramipril with Ibuprofen?

NSAIDs like ibuprofen oppose the effects of ACE inhibitors like ramipril in two ways: they cause salt and water retention that raises blood pressure, and they constrict the blood vessels supplying the kidneys. In someone also taking a diuretic (the 'triple whammy' combination of ACE inhibitor + diuretic + NSAID), this can cause sudden kidney failure. An occasional ibuprofen for a headache is generally fine; regular use should be avoided in favour of paracetamol.

How do Ramipril and Ibuprofen interact?

Renal perfusion in patients on ACE inhibitors depends on prostaglandin-mediated afferent arteriolar dilation, especially when renin-angiotensin activation is present. NSAIDs block this compensation, reducing GFR. NSAIDs also cause sodium retention, antagonising antihypertensive effect by several mmHg on average.
